Support add-ons on fields with floating labels (#876)

Bootstrap nests .form-floating inside .input-group, next to the add-ons,
rather than treating the two as alternatives. is_floating no longer
excludes fields that set addon_before or addon_after, and form-floating
moves from the field wrapper to a div inside the input group when
add-ons are present.

This removes the warning added in 26.3 (#833), which existed only
because the combination was unsupported. Widgets that cannot float are
unchanged: a color input with an add-on still renders its label above a
plain input group.

Fixes #866.
