PWA: drop the keyboard-placement experiments, keep the layout fixes

Revert the floating Save bar to the placement that worked before the form
outgrew the screen: fixed, offset by window.innerHeight - offsetTop -
visualViewport.height. Measuring from documentElement.clientHeight and
hanging the bar in a document-coordinate layer both placed it further off on
iOS, and neither can be told apart without readings from a device, so the
proven formula stays.

No recurring work is left behind: no polling, no animation-frame loop. The
keyboard height is read on visualViewport events, on orientation change, and
three times right after focus, because iOS does not always send a final
resize once the keyboard has settled.

Kept: the single scroll area that stops the main Save bar from scrolling off
screen, the dynamic viewport height so a rotation re-measures, and the DEV
banner moved into the layout flow — pinned over the page it pushed the header
down onto the content.
