fix: surface operator reasons and right-size ASOF workspace charges (#299)

DAL-288: streaming error projection collapsed every OperatorReason into
the same opaque "operator ... execution failed" text, and ASOF admission
accounting charged a per-row schema tax of roughly 41 KB, so a
10,000-row batch needed about 410 MB against the 64 MiB limit the docs
recommend.

- projection: append the engine-authored OperatorReason message and
  reason code to the node-prefixed failure text on the entry, task, and
  preflight surfaces; other errors keep their existing fallbacks.
- admission: replace the schema tax with an allocation-free arithmetic
  upper bound on each admitted row's bounded IPC encoding (repeated
  schema-message bound from field sizes and metadata, slice bytes with
  IPC framing and alignment headroom) and reduce identity charges to an
  aligned slice bound plus node headroom. Charging stays arithmetic so
  an unfittable schema is rejected before any flatbuffer is built, while
  max_state_bytes still bounds the workspace before any encode.
- regressions: convert the DAL-287 repro into dual-path tests
  (convenience stream and explicit StreamingRunner bindings); 10,000-row
  batches now pass under AsofStateLimits(100_000, 64 MiB) and forced
  workspace-limit failures surface the message and reason code.
