Fix non-breaking space normalization to avoid corrupting code content

The previous fix (385126b) did a blind string replace of ' ' on
the final Markdown output, which would also corrupt legitimate literal
text like ` ` inside a code span or block (e.g. a post
documenting the entity itself).

Instead, walk the DOM before conversion and replace real U+00A0
characters with a regular space in text nodes, skipping anything
inside <code>/<pre>, so only actual non-breaking spaces in prose are
normalized.
