docs: document CreateDirAll, StatusCode and AlreadyExists

The v1.2.1 additions were released without reaching either README, which
claims its samples track the current API. Adds CreateDirAll next to
CreateDir with the os.Mkdir and os.MkdirAll comparison that explains the
difference, and extends the error-handling section with the StatusCode
field and the AlreadyExists helper.

Also removes the em dashes, aligns the tables and adds the shared
markdownlint config. The samples were compile-checked against v1.2.1.
