importer: decouple layer extraction from the registry

processLayer had too many arguments and reached straight into an
ImageSource and a blob cache, so the tar walking, whiteout handling and
zip-slip guard could only be exercised through a real OCI archive
fixture.

Move it to a fileExtractor that is handed a blobOpener, a function
yielding a layer's bytes. The extractor now owns the whole layer walk
and becomes orthogonal to registry logic.

Add tests to exercise skipping whiteouts, directories, unreadable
layers, and path-traversal checks.
