feat(csp): log the full report and page URL for CSP violations (#34)

LogCspViolation dropped every field except effectiveDirective and
blockedURL. The page URL was only in the log context, which some
Monolog handlers do not forward.

Put the page URL in the message and the full raw report in the
context under 'report', the same as LogReport does. The existing
'page' context key is kept.
