make the custom command failure panel opt-in per command

#613 enabled failure panels for every custom command. Panels are now off by default. A command turns one on with `--error-hud` in `keymap.conf`, after the optional chord or right after the name for a palette-only command, placed with `--error-position POS` (default center) and `--error-pane left|right` (default the whole session). A configured pane that is hidden or gone falls back to the whole session at the configured position. macOS banners are unchanged, and opted-out commands create no stderr capture file.

The flags are parsed as a prefix that ends at the first ordinary shell token or `--`, which also escapes a shell body starting with a reserved name. Unknown leading `--error-*` options, repeated options, missing or invalid values, and placement without `--error-hud` are diagnosed and skip the command before anything spawns. `keymap list` reads all three back.
