chore: Ignore Claude Code worktrees and local settings (#604)

Worktrees are created under .claude/worktrees/, so they showed up as
untracked files in the repo. Local settings are per developer and should
not be shared.

.claude/settings.json and .claude/agents/ stay tracked so shared config
keeps working.

Add .worktreeinclude with .env, so a new worktree gets the local env file
copied in instead of starting without one.
