restore coverage lost to recent fixes

Cover the branches recent fixes left untested, mostly an in-flight
async load whose timestamps read expired, and remove the checks that
no path can reach. A malformed jcache listener path now fails like
any other setting instead of hiding the cache, put's duplicate
putIfAbsent fast path is merged, and CacheProxy's expired-read
removals share one helper.
