* fix: accept ESM-only PostCSS plugins in the postcss transform

postcss-load-config v3 loads plugins with `require`, so an ESM-only
plugin such as postcss-nested v8 comes back as a module namespace object,
which PostCSS rejects. Unwrap its default export before passing the
plugins to PostCSS.
