Handle Content-Type and Content-Length headers per the Rack SPEC

The Rack SPEC requires CONTENT_TYPE and CONTENT_LENGTH to appear in the
env hash without the "HTTP_" prefix; every other header keeps the prefix.
The Rack adapter previously prefixed all headers uniformly with "HTTP_",
so requests carrying Content-Type or Content-Length surfaced as
HTTP_CONTENT_TYPE / HTTP_CONTENT_LENGTH, which Rack apps do not read.
