feat: add swPlane field to RailTopology and tag uplinks with plane_id

Adds an optional swPlane field to SpectrumXRailPoolConfig.spec.railTopology[]
so HW-PLB rails (multiple pfNames) can identify which switch plane they
belong to. Each xplane uplink port now carries an extra
external_ids:plane_id=<P> where P = len(pfNames) * swPlane + pfName index,
so downstream tooling can distinguish uplinks across planes.
