docs: correct the coexistence claim for revdiff-planning and cc-thingz planning

both plugins register a PreToolUse hook on ExitPlanMode and claude code runs
plugin hooks separately, so enabling both can review the same plan twice. the
old wording promised no conflict. README and site/docs.html now carry the same
sentence; docs.html previously omitted the cc-thingz example.
