feat(mediators): Report the block an import is stalled at

Import now stops at a block whose batch could not be retrieved or
applied rather than moving past it, so a batch that stays unretrievable
halts confirmation of every later block until it resolves -- and nothing
said so. A gauge names the stalled height, 0 when clear, and the stall
is logged with the block and its error, so an operator can see the node
has stopped confirming and why.
