Keep the production cloud address in one place

- drop the class-level cloud_agent_url literal: __init__ always derives it
  from cloud_base_url, so the literal could only ever drift out of sync
- build the built-in URL from PRODUCTION_PROVIDER_NAME, since a provider is
  named after its host
