fix(views): keep title-less paragraph menus aligned at the right edge

A title-less paragraph floats its row menu to the top-right corner. The
first-heading spacing rule (1.25em top margin) outranked the title-less
rule by specificity, so the first block's menu dropped down, spilled into
the next block and pushed that block's menu leftward out of line.

The first-heading margin now applies only to titled headers, each
paragraph block contains its own floats (flow-root), and the title-less
menu gets the same 8px top margin as the content so the chevron rides the
first text line.
