chore(cli): load runtime settings through Effect Config (#6624)

Runtime settings read the process environment directly, so an injected
Effect configuration provider could not control profile selection,
credential storage settings, or telemetry configuration. Load these
values through Config and install a live environment provider at CLI
composition.

Keep the existing flag, environment, and persisted-profile precedence,
including empty-value behavior and build-time telemetry defaults. Login
consumes the optional environment profile and resolved storage path from
CommandSettings, so profile selection and persistence use the same
configuration.
