Clean up creator name handling

**ISSUE**
Inconsistencies in creator name handling were resulting in the
`ExportJob` CSV files omitting creator names.

**RESOLUTION**
This change extracts creator name normalization (i.e. removal of
parenthetical names, birth, and death dates) to its own class
that both `ExportJob` and `CitationFormatter` depend on.

This change also cleans up `Creator` lookup from IDs stored on works.
