bedrock-22g: address cold-audit notes

- materializer_bootstrap_phase: say why recovery's hd() stays — it
  seats one worker, tag 0, on a fresh cluster, and a singleton has
  nothing to spread itself against.
- server: name the placement cost of the deliberately-ambiguous
  commit-timeout branch (a live worker no member and no reservation
  accounts for).
- server_test: the tie-break case now discriminates on the in-flight
  reservation (it resolved the same either way before), and the
  no-capable-node case pins that no worker is created.
