fix(metrics): register prometheus histograms

Backport of #506 to v2. PrometheusReporter built every HistogramVec but
never handed the map to prometheus.MustRegister, so ReportHistogram
observed into unregistered collectors and no histogram ever reached
/metrics. Registers the histogram map and drops the built-in
response_time_ns histogram, which nothing called and would collide with
the summary of the same name once registered.
