claude: extend the prose rules and apply them

`spell` joins the banned verbs. `spell-check` is exempt in every form
and position. Two patterns join the phraseology table. `X verbs no Y`
is rewritten as `X does not verb Y`, and `X states Y` is rewritten as
`X requires Y`. `claim` gains an exemption wherever a specification
defines it as a term, such as an OIDC or JWT claim.

The prose-lint greps cover the additions. The banned-verb grep gained
the `spell` inflections, and the phraseology grep gained one
alternation for each new pattern.

The other edits bring the existing documents into line with the rules
above. The line length entry in `general.md` also becomes exact. The
old wording excluded a line of exactly 100 characters, a length every
formatter and linter here allows.
