feat(incidents): add query and policy view contracts (#326)

  Root cause: fact policy and derived backlinks lacked server-qualified provenance, while read views shared mutable storage members.
  Fix: bind scoped facts, track event-qualified backlinks, and filter list and similarity results on detached current-policy views.
- f5bd63e9132e fix(incidents): qualify list filters by project scope
  Root cause: list filtering carried only a project ID, so membership and artifact checks could cross-match different stores, projects, or environments.
  Fix: carry the full request scope through candidate and public queries and require exact scope matches for project membership and backlinks.
- 31b8b6c7f103 fix(incidents): separate incident and project stores
  Root cause: API adapters inferred source-project provenance from the incident store route, which fails under dedicated or application incident storage.
  Fix: require server-resolved project scopes for create and list conversion and name the provider filter store explicitly.
- 76a64ed5c5d7 fix(incidents): validate resolved project stores
  Root cause: list adapters used generic filter validation that accepted non-canonical resolved project store IDs.
  Fix: apply the canonical fact-scope validator before query construction and at the provider candidate boundary.
