Preserve root causes in index database errors

Retain the full anyhow error chain when recording whole-database index failures so partial-failure logs and failed-scrape errors include the database context and underlying cause.

Add a regression test verified to fail with the old Display-only formatting and pass with alternate formatting. Correct the obsolete collector name and index module layout in AGENTS.md, and document the diagnostic fix in the changelog.

Validation: cargo fmt --all -- --check; cargo clippy --all-targets --all-features; full cargo test against local DevPod PostgreSQL 18; index_scrape at concurrency limits 1 and 2. The three container-only tests remain skipped locally.
