Report coverage to Coveralls instead of asserting it

A hardcoded 100% in the README was a claim about the build, not a
measurement of it. The badge now comes from Coveralls, which gets the
real number from the same combined data the gate reads.

tox writes the lcov after `coverage report --fail-under=100`, so a run
that drops below the gate uploads nothing and the badge keeps the last
honest figure rather than quietly recording the regression.

The test that tied the badge text to the gate goes with it. There is no
number in the README left to drift.
