Fix Dialog swap showing both dialogs on double-click

A rapid double-click on a swap trigger opened both dialogs. The second
click landed on the just-opened dialog's own swap trigger before its
entry transition ended. The swap opened the next dialog, but the
still-transitioning dialog refused to hide, so both stayed open with a
doubled backdrop.

Ignore a swap trigger while the outgoing dialog still carries
.dialog-swap-in, which marks an unfinished entry transition.
