feat(ws-connector): forward both island_changed subjects without a flag

Drop LEGACY_ISLAND_CHANGED_FORWARDING. The five-token subject
(engine.peer.{wallet}.island_changed.{session}) always goes to the socket
registered under that session, and the legacy four-token subject always goes
to the wallet's newest socket, so deploying ws-connector before
comms-gatekeeper is the only transition requirement. Docs and the
integration spec follow the flagless behaviour.
