docs: state the TERM_PROGRAM=agterm identity and the hyperlink workaround

agterm sets TERM_PROGRAM=agterm and TERM_PROGRAM_VERSION on every surface since #203, and the environment reference omitted it. Claude Code decides OSC 8 hyperlink support from a terminal-name allowlist that lacks agterm and prints links as plain URLs.

document the identity pair, the FORCE_HYPERLINK=1 override per command and through env in the scoped ghostty.conf, and the precedence that makes an env line naming an injected variable a no-op.
