* chore: add goveralls to install-tools and a make coverage target

* test: add tests for stack, ternary, and cobra_x_error to raise coverage

Raises internal/utils from 43.8% to 93.8%, internal/utils/ternary from 0% to 100%, and pkg/types from 38.9% to 100%. Overall coverage goes from 96.49% to 97.40% in the Coveralls line metric.

* docs: add unreleased changelog entry for coverage tests

* chore: add make goveralls target for local coveralls report
