Hide the extract button when extraction is disabled

With extraction.enabled false the article card still offered "Extract
Content". The handler answers that request with a JSON 500 saying extraction
is disabled, and htmx does not swap error responses, so the click did nothing
visible and the message reached the server log only.

The card now offers the button only when extraction is enabled, the empty
content placeholder says extraction is disabled instead of pointing at a
button that is not there, and the README describes what the UI does.
