refactor: StackMachine::emit; split remaining visitor and machine files (#322)

AssemblyGenerator::emit was a pass-through switch. The Op dispatch is now
StackMachine::emit, which also finishes the instruction. The generator
only walks the IR.

Compare, assign, unary, add/sub and shifts move to StackMachine_Arith.
addressOf, functionAddress and dereference move to StackMachine_Addressing.
nativeMoveSize is one private inline.

CodeGeneratingVisitor expression visits go to _Expressions, declarations
to _Declarations. FunctionDefinition and Block follow after valueFromSymbol
moves next to addFrameTemp. The main visitor file is emit/id/convert only.
