Start moving documentation to GitHub Pages (#70)

Converted the Subscriptions and Pointing Reference Frame model
documentation from binary formats to reStructuredText. Also moved most
things from the README, coding standards, and other documentation pages
to GitHub Pages.

This is the first of many PRs which will slowly transition our model
documentation away from binary formats to text-based formats.
