docs: describe the full session env strip, not just CLAUDECODE

PR #430 replaced the bare CLAUDECODE strip with sessionEnvVars, but README,
llms.txt, CLAUDE.md and the ralphex skill file all still said only CLAUDECODE
was removed.

Names three markers with a pointer to the full list rather than duplicating all
ten, scopes the claim to the claude and codex child processes since the custom
review runner still inherits the host env unfiltered, and states that marker
names are matched exactly so CLAUDE_CODE_USE_BEDROCK style config vars survive.
