refactor: remove session state (#1474)

Removes the per-client `session` object as a first-class concept. The
server is now request-scoped and stateless: every tool call is driven by
its request (args, headers, `_meta`), the static `UserConfig`, and
app-level shared services — no server-side per-client `Session` object
held in memory. Per-client state (MongoDB connections, exports,
identity) is either keyed by `connectionId`/client identity or carried
on the request.
