fix(diagnostics): fan-out repair advice names the projection APIs 2.0 ships (#3304)

WIDE_WRITE and HOT_SCOPE_FANOUT messages, both agent skills and the
diagnostics doc pointed at an API that is not in 2.0; guidance written
for coding agents produced a failing import. They now prescribe a
projection: createProjection, or a createStore(fn) keyed by id.
