Fix AndTrigger.max_iterations type to match its actual behavior (#1136)

Passing max_iterations=None constructs fine but blows up with a confusing TypeError the first time next() calls range() on it. The iteration cap is there on purpose (it's what backs MaxIterationsReached), so None was never actually usable, just untyped. Narrowed the annotation to int and added a test that locks in the TypeError so nobody quietly turns this into an unbounded loop later on.

Fixes #1131.
