Align with chubbyphp-trusted-proxy

- resolve nothing without a remoteAddress attribute,
requireRemoteAddress (third argument, config key) disables the check
- strip the port from the remoteAddress before matching the trusted
proxies
- scheme must be http or https, host must be a syntactically valid
host with an optional port, both lowercased
- clientIp: ipv4 mapped ipv6 addresses as ipv4, zone ids are not valid
ips and never trusted
- headers: each key optional and replacing its default, null disables
proto and host
- service factory: validate headers and requireRemoteAddress, prefix
the config path to resolver errors
