ci(workflow): prevent duplicate coverage runs racing on Coveralls

Push and pull_request events both triggered a full matrix run for the
same commit on PR branches; 20 concurrent goveralls reports raced and
Coveralls closed one build while stragglers still reported, failing
with 422 "Can't add a job to a build that is already closed".

- restrict push triggers to master, so PR branches only report via the
  pull_request event
- add a concurrency group to cancel superseded runs on rapid pushes
