fix(rsa): validate hash availability and ciphertext block alignment

- return a regular error instead of panicking when a crypto.Hash value has
  no implementation linked into the program (crypto.Hash.Size/New panic)
- reject ciphertext whose length is not a multiple of the key size up front
  in RSADecrypt and RSADecryptOAEP for a clearer error on malformed input
- clarify the RSAGeneratePublicKey doc comment: DER payload is PKIX
  SubjectPublicKeyInfo, the "RSA PUBLIC KEY" PEM label is kept for
  backward compatibility
