ci: actualize GitHub Actions workflow

The baseline workflow can no longer run: ubuntu-18.04 runners are
retired, actions/upload-artifact@v2 is disabled on github.com, and
coverallsapp/github-action@v1 is unmaintained. Actualize it while
keeping the change minimal and the whole matrix green.

- bump actions to current node24 majors: checkout/setup-node@v6,
  upload-artifact@v7, download-artifact@v8, coveralls@v2
- run on ubuntu-latest instead of the retired ubuntu-18.04 image
- restore the baseline Node.js matrix (10-16 on linux, windows and
  macos on 16.x; Node <16 has no arm64 macOS builds)
- artifact actions v4+ are immutable: upload one uniquely named
  artifact per matrix job and download them by pattern with
  merge-multiple, keeping the flat layout nyc merge expects
- coverage path normalization: replace the 'replace' CLI, which is
  not a dependency of this project, with a sed one-liner
- coverage job: setup-node without an explicit version no longer
  works (v1 default is gone), so pin node 16.x; coveralls v2
  auto-detects the lcov report file
