test: cover the enrolment controller, 0% to 50% (#4)

The plugin's front door and its largest single file, previously with no
test at all. Everything behind it assumes the caller is a member's
handset, and this is the only code that decides so.

Most of what is asserted is a refusal, and three are easy to break with
nothing failing: the five-device cap, the shared enrolment path -- so a
password sign-in cannot skip a check the OAuth one makes -- and the fact
that a revoked handset is refused because it can no longer be found
rather than because anything downstream inspects a flag.

The cap is read off the class by reflection rather than restated. A test
carrying its own copy of 5 goes on passing when the cap moves and stops
testing the cap.

Two of these were wrong first time and both were worth having: the
member double takes its members through the constructor rather than an
add(), and the exchange reads its identity from the code store rather
than from the provider -- so the stranger had to be put in the code, not
in a stub.
