Resolve nothing for a broken remote address instead of falling back to the headers

A remoteAddress attribute or REMOTE_ADDR server param which is set but
not a string (int, array, object) counted as not set, so with
requireRemoteAddress disabled the headers got used. It now counts as set
but invalid, like a junk string: nothing gets resolved, the headers get
ignored.
