Modernise and tighten `.gitignore` (#16850)

* Modernise and tighten `.gitignore`

The large amount of any-depth rules in `.gitignore` recently caused a
bug in a wheels-build update[^1] where a file in
`.github/workflows/wheels` didn't get committed (and we failed to run
the optional tests in CI that would have caught it).  I took this
opportunity to remove a lot of ancient dead weight from the file, and
tighten up all the remaining rules to (where appropriate) target precise
paths from the repo root, not arbitrary-depth ones.
