fix(velocity): cancel pending premium verification after a failed forced online-mode login

Velocity fires no event when Mojang rejects a forced online-mode login,
so a player who ran /premium and then reconnected from an unlicensed
client stayed in pendingPremiumUsernames forever and was forced into
online mode on every reconnect. The pendingVerificationAttempted marker
from 7b52ffb (#3051) was lost in the premium verification refactor, and
clearPendingPremiumVerification in the Velocity bridge became dead code.

Restore the marker in VelocityPremiumVerificationManager: the first
PreLogin of a pending player forces online mode, a second one while
still pending means the previous attempt never completed, so the pending
entry is cleared and the player gets the normal password login. The
marker is reset on successful verification, on premium.pending.set and
on disconnect. Enrolled premium players keep being forced into online
mode regardless.
