Add SimpleCov and Coveralls integration, bring coverage to 100%

Excludes unused Rails-generated scaffolding (ActionCable, ActiveJob,
ActionMailer, ApplicationRecord) from coverage since this app only
mounts a Grape API and none of that scaffolding is exercised.
