Update VS Code LLM integration (#6492)

Configure VS Code and VS Code Insiders through chatLanguageModels.json
using the customendpoint provider and models discovered from the
authenticated gateway.

Add placeholder proxy authorization, preserve existing configuration,
clean up obsolete Copilot settings, and handle unsupported lazy setup
semantics.

Fixes #6294
