coverage: hide internal error paths from lcov

Mark internal errors with:
/* INTERNAL_ERROR_EXCL_START */
/* INTERNAL_ERROR_EXCL_STOP */

These errors signify bugs in FORM and errors reading/writing from files, etc.
In short, one could not reasonably be expected to write test cases which
verify the error message. Marking them with the exclusion markers makes this
clear in the code.
