docs: explain the repeating App Data permission prompt (#537)

macOS App Data consent is held by a running process rather than stored as a
setting. A pane carried across a restart by Live sessions mode was started by an
agterm that has since exited, so every process in it answers as its own
responsible process and the prompt returns on each command. Full Disk Access is
the only permanent answer.

Also corrects the claim that agterm ships no folder usage-description strings —
it ships Desktop, Documents and Downloads — and adds NSAppDataUsageDescription
so the dialog carries agterm's own wording.
