Alert on totally-failed qualification batches

A dead API key fails every row inside qualify_pending's stats, which the
report node never saw — 50/50 failures produced no Slack alert while the
hunt looked healthy (observed live: rotated OpenRouter key -> 401 on all
rows, silence). A batch where errors == fetched now escalates into graph
error state with the first error message, so the existing report alert
fires. Partial failures remain informational.
