fix: render a row per stored webhook header

The webhook form rendered a fixed 2 header rows. `unredact_headers/2`
rebuilds the stored map from the submitted headers only. A backend with 3
or more headers therefore lost the extras on a UI edit, and Elixir map
order decided which 2 survived.

The form now renders one row per stored header, sorted by key, plus one
blank row for a new entry. It keeps a minimum of 2 rows, so the form for a
new backend is unchanged.

`transform_params/1` derives the header indexes from the submitted params
instead of a hardcoded 1..2 range. The OTLP form and the Loki header row
are unaffected; they submit the same key shape.
